1. THE COLLEGE AND FACULTIES’ NEW OFFICE IN NEW ZEALAND

The New Zealand office of the College and Faculties relocated its premises in July to Level 7, Exchange Place, 5 – 7 Willeston Street in the CBD in Wellington.  The new office was officially opened on Friday 23 November by the President of ANZCA, Dr Walter Thompson.

The ‘opening’ cocktail function provided an opportunity to promote the role of the College and Faculties in providing training, education, standards and research in anaesthesia, intensive care and pain medicine. 

Guests included Fellows and trainees and representatives from Government and non-Government agencies with whom ANZCA, JFICM and FPM interact with in New Zealand.  The Hon Mita Hirinui, Associate Minister of Health, represented the Prime Minister, the Rt. Hon Helen Clark.  The Mayor of Wellington, Her Worship Kerry Prendergast also attended the opening.

The new office is closer to many of the agencies we interact with, compared with the previous office location in Elliott House. The office provides good IT and staff facilities and space for meetings and workshops.  Exchange Place has a pleasant outlook across the waterfront and harbour.

Fellows and Trainees are welcome to use the facilities when in Wellington.  The New Zealand National Committee and staff look forward to welcoming you to the National Office in Wellington.

2. 2007 HIGHLIGHTS

As well as the relocation to the new office, the New Zealand Committee has had a wide range of issues to consider and progress during 2007.  These have involved interactions and meetings with ANZCA Council, committees, Fellows and trainees, Joint Faculty of Intensive Care Medicine and Faculty of Pain Medicine as well as with ‘external’ organisations and individuals such as the Medical Council, Ministry of Health & DHBNZ, Council of Medical Colleges, Director General of Health, Health and Disability Commissioner, Chief Coroner, NZSA, NZ Anaesthetic Technicians Society, Nursing Council and Pharmac.

The New Zealand Committee of ANZCA has considered many ‘internal’ and ‘external’ discussion documents over the last twelve months.  Over thirty submissions were prepared.  Topics covered a wide range of anaesthesia practice and New Zealand health care issues, such as:

All the College work could not be done without the voluntary commitment from ANZCA Council and committee members, examiners, assessors, the ASM committees and the dedication of supervisors of training, clinical directors and others in the anaesthesia departments and ANZCA staff.  My thanks to everyone who has strived to ensure the training, education and standards for anaesthesia serve the public of New Zealand well.

Some of the 2007 and future activities are described in more detail below.

3. ANZCA NEW ZEALAND NATIONAL COMMITTEE

The New Zealand National Committee (NZNC) last met in Wellington on the 23 November.  The President of ANZCA, Dr Wally Thompson, the new Director of Professional Affairs, Dr Steuart Henderson and the Director of Policy, John Biviano attended this meeting.  Dr Mike Richards, CEO of the College joined the afternoon session.  The opening of the new office followed the meeting.

2008 NZNC Meetings

NZNC has three scheduled meetings for 2008: 28 March, 25/26 July and 21 November.

NZNC Elections

2008 is an election year for NZNC.  Call for nominations and the election details will be sent out to all NZ Fellows in March.  NZNC work is interesting and rewarding, so consider putting your name forward.

4. RECENT APPOINTMENTS

ANZCA Director of Professional Affairs (DPA)

Dr Steuart Henderson has been appointed as an ANZCA DPA to undertake primarily the Assessor role and to have responsibility for reviewing and updating the Regulations for Council.  He will contribute to the work of NZNC regarding New Zealand issues of interest to ANZCA.  Dr Henderson is a past-Council member, based just outside Wellington, who served as Assessor and Chair of the Education and Training Committee during his term of office.

Medical Training Board

Dr Malcolm Futter, FANZCA, has been appointed by the Minister to this newly formed Board which we hope will bring some sense to the workforce planning discussions.  Malcolm Futter has held key positions within the New Zealand National Committee, as Chairman of the New Zealand National Committee, National Education Officer and New Zealand Assessor.  He has also been an examiner for ANZCA and for the Royal College of Anaesthetists in the UK.

During the period of 1989 to 2005, Malcolm Futter was Chairman of the Anaesthetic Vocational Training Committee for the Auckland Region and the ANZCA Regional Supervisor of Training for the northern rotation in New Zealand. 

Malcolm Futter is currently the Interim Director of Capital and Coast DHB Anaesthesia, Intensive Care and Pain Medicine.

Director of Education

Medical Educator, Ms Mary Lawson, will join the ANZCA head office staff in February to take up her appointment as Director of Education.  Ms Lawson’s experience in both clinical and educational environments in Australia and the UK, and her research activity, will be of great relevance to her new position.

5. EXAMINATIONS AND TRAINING

  Examination Successes

Congratulations to all the successful 2007 exam candidates.  Forty-seven New Zealand trainees passed the final exam.

Final Examination Venues

In 2008 the written/clinical examinations will be held in Brisbane, Sydney, Melbourne, Adelaide, Perth, Auckland and Hong Kong.  The vivas will be held in Melbourne (May) and Sydney (October).

Dates for the 2008 Primary and Final exams can be found on the ANZCA website at http://www.anzca.edu.au/trainees/examination-dates/.  The new ANZCA website has a great amount of useful information on the FANZCA training.  Trainees have complimented staff on this improved site.

6. ANZCA MOPS/NEW CPD PROGRAM

NZNC submitted the new ANZCA CPD Program to MCNZ for approval and this was confirmed in December.

The CPD Program began on 1st January 2008, replacing the MOPS Program.  It is a Triennial rather than Annual program.  All MOPS participants were sent a hard copy of the CPD Program booklet.  It is also available electronically on the ANZCA website: http://www.anzca.edu.au/news/announcements/cpd-launched.html

People will find it useful to print out the Framework Table from the booklet (when printing from the PDF booklet, specify pages 7 and 8) as this gives an ‘at-a-glance’ run-down of types of activities, examples and the credits to claim.  In fact, participants should be able to follow the Program using just these two pages, in much the same way as the whole MOPS booklet was used for the MOPS program.  (The remainder of the CPD Booklet is more for reference or for the Toolkits when participants need them.)

CPD Portfolio

The online and offline versions of the CPD Portfolio are now available on the website.  All the relevant web pages, text boxes or forms can be accessed online.  These items will make up participants’ CPD Portfolios, along with any hard copies of supporting documentation for activities undertaken during the triennium.  Currently ANZCA is aiming to have a facility for online storage for documents (e.g. emails confirmations and PDF documents) as a part of the portfolio.

Transition to the new CPD Program

The Toolkits have been developed in order to assist Fellows with each element of the new Program. These should be read carefully – if adhered to, participants should be able to meet the specific requirements of the Program with little difficulty.

Record-Keeping

It would be useful for participants if attendance lists or certificates for departmental meetings, or workshops, and so on, are made available. Supporting documentation such as this should be retained by Fellows throughout the triennium.

Fellows should be prepared for participation in the random annual audit, by ensuring that their supporting documentation is compiled appropriately.

Departmental Activities

The Objectives of the ANZCA CPD Program include:

  • To promote and facilitate the participation of Fellows in effective CPD within the scope of their practice

As the new Program focuses on individual practice, it would be appropriate for departments to facilitate activities that are relevant to the individual’s requirements.

  • To encourage a culture of self-directed learning, review, and reflection on professional practice among Fellows;

Departments may decide to organise meetings in which Fellows may focus on reflection of their professional practice.  Whilst evaluation should be an ongoing activity through the triennium, in the final year ‘evaluation of participation’ is an activity for which Fellows may claim 3 Credits per hour (under Category 3 – Practice Assessment Activities, Level 2 – Self-Directed QA Activities).

7. PERIOPERATIVE MORTALITY REVIEW COMMITTEE ESTABLISHMENT

Recently ANZCA representatives met with the Director General of Health, Stephen McKernan and Ministry officials (Dr Dave Galler and Gillian Bohm) to progress the idea of establishing a Perioperative Mortality Review Committee.  The Ministry now seems in favour of progressing this. ANZCA is currently gathering more information to help move this idea forward. 

8. NZ ANAESTHESIA EDUCATION COMMITTEE (NZAEC) NEWS

NZ Anaesthesia ASM - Auckland

On behalf of the New Zealand National Committee of the ANZCA, I congratulate and express thanks to Helen Frith, Alan McLintic and the others on the organizing committee from Middlemore Hospital for bringing together such an exciting programme in Auckland in November for the combined scientific meeting of the New Zealand Committee of ANZCA and the New Zealand Society of Anaesthetists. 

The programme based on the theme, ‘Facts, fads and folklore’ challenged and entertained the large number of attendees who were able to consider how the evidence presented impacts on how we deliver safe and effective anaesthesia. 

An exceptional faculty of international and local experts addressed this issue over a broad range of topics.  In addition, throughout the meeting, there were thought provoking contributions from scientists outside anaesthesia, including world authorities on global warming and sports doping.

My thanks also go to the New Zealand Anaesthesia Education Committee (NZAEC) and Rose Chadwick for providing guidance to the organisers of the ASM.  A number of NZAEC led meetings during the ASM allowed:

NZAEC Chairman

I wish to congratulate Dr Ross Kennedy who has recently been appointed Associate Professor of Anaesthesia in Christchurch, an honour which recognises his long interest in research, education and teaching.  He has relinquished the position of Chair of NZAEC following his resignation from the NZSA Executive. Ross has made a significant contribution to NZAEC and will continue to use his expertise as convenor for the 2010 ANZCA ASM in Christchurch.  We wish to thank Ross Kennedy for his commitment and leadership.

NZAEC recently elected Dr Brian Lewer as the new Chair of NZAEC.  Brian is a member of ANZCA NZNC and is one of the NZNC representatives on NZAEC.  Congratulations to Brian on this appointment.

BWT Ritchie scholarship

The BWT Ritchie Scholarship selection committee has awarded two Scholarships for 2008, to Dr Nick Abbott and to Dr Rachael Glew.

Dr Nick Abbott is completing his provisional fellowship at Middlemore Hospital this year and has been offered a fellowship position at the Alfred Hospital in Melbourne next year.  Dr Abbott is particularly looking forward to developing his skills in the areas of transplant anaesthesia (especially heart and lung, as well as ventricular assist devices) and major trauma, and to consolidating his skills in anaesthesia for otolaryngology, facio-maxillary and airway management.  Dr Abbott has identified several CME meetings in Australia that he is interesting in attending while there.

Dr Rachael Glew has recently completed her fellowship in Liver and Cardiac Anaesthesia in Auckland and has secured a Clinical Fellowship in the ICU at Middlesex Hospital in London.  A key focus for Dr Glew will be finding out more about the use of Transoesophageal Doppler in Goal directed therapy.  Dr Glew notes that being located in a busy central London hospital will provide exposure to a wide and interesting case mix, with an opportunity to gain invaluable experience in perioperative and medical management of critical patients.

NZNC congratulates this year’s two Scholarship winners and invites other trainee anaesthetists to consider applying in 2008.  The Scholarship is open to New Zealand trainees who have passed the final examination for the Diploma in of Fellowship of the ANZCA or in JFICM, and who wish to pursue a further year of study outside New Zealand in their final year of training, or the year following the completion of their Fellowship. 9. REPORT FROM THE HEALTH AND DISABILITY COMMISSIONER

NZNC received a copy of the following anonymised report prepared by the Health and Disability Commissioner's office on a case concerning inappropriate post-operative care by an anaesthetist, and misplacing of patient dentures causing respiratory arrest.

The case has been referred to us for the education of anaesthetists in New Zealand.

Summary of the event

The Health and Disability Commissioner received a complaint about the services provided by a private hospital to an elderly patient who was admitted for elective spinal surgery. The investigation was later extended to include both the orthopedic surgeon and the anaesthetist with regards to the appropriateness of the care and treatment provided.  The Commissioner found that Right 4(1) – the right to services provided with reasonable care and skill - had been breached by the anaesthetist by failing to provide the patient with an appropriate standard of anaesthesia care in the immediate postoperative period.  No breach was found for the private hospital or the surgeon.  It was recommended by the Commissioner that the anaesthetist review his practice and apologise to the patient’s family. 

Induction of anaesthesia was uneventful and the patient’s upper dental plate, being noticeably loose, was removed prior to intubation.  Anaesthesia, positioning of the patient and surgery were all straightforward and the anaesthetist decided to recover the patient in the recovery room as it was the last case of the day. The patient was transferred with an endotracheal tube in situ attached to a T bag, his vital signs were satisfactory and oxygen saturations were maintained with assisted ventilation.  However he was slow to breathe and was given naloxone by the anaesthetist in recovery. The anaesthetist left the patient for approximately 5 minutes around this time to check on patients operated on earlier in the day.  The Commissioner was concerned that “the anaesthetist left the patient under the care of a nurse who was not comfortable accepting responsibility for a patient who she considered had not yet fully recovered the ability to breathe.”

The patient was extubated by the anaesthetist and returned to the ward, where his denture was still noted to be missing despite efforts to locate it.  His respiratory condition subsequently deteriorated and he was treated for fluid overload and bronchospasm with limited improvement. Three days after his operation, the patient developed respiratory distress and subsequent arrest and on reintubation, the upper denture was discovered lodged in his pharynx.  He was successfully resuscitated and transferred to a public hospital intensive care unit and was later transferred to another public hospital for rehabilitation. An MRI scan revealed a syrinx from T4 to the medulla and muscle wasting of his tongue, which were reported as long-term changes.

Despite extensive investigation into the private hospital it was not possible to establish who replaced the dentures or when this occurred. A serious underlying and undiagnosed neurological condition (syringomyelia) allowed the patient to tolerate his dentures in his pharynx leading to aspiration pneumonia, respiratory distress and ultimately arrest.  The patient never fully regained his previous good health and was unable to continue living independently.  He died two years after surgery from a pneumonia related condition.

10.  REVIEW OF THE HEALTH PRACTITIONERS COMPETENCE ASSURANCE ACT

The NZNC submission covered many aspects of the Act. 

The purpose of the Act is to protect public safety.  The Act is partially achieving its purpose. 

The regulatory authorities are good at identifying and enforcing the entry requirements into a given professional group for practitioners training within New Zealand.

From ANZCA’s perspective there are several areas where the implementation of the current Act fails to ensure its purpose is achieved.  The main areas of concern that were highlighted in the NZNC submission were the need to improve the processes regarding:

  1.  Registration of International Medical Graduates (IMGs)
  2. Regulation for new groups of registered health professionals
  3. Overlapping Scopes of Practice - failure to ensure that overlapping Scopes of Practice provide the same standard of care
  4. Ensuring competence.

11.  NEW ZEALAND ANAESTHETIC TECHNICIANS  – REGULATORY BODY

The previous Minister of Health, Pete Hodgson agreed that anaesthetic technicians are to be regulated as a health profession under the HPCA Act.  A decision is awaited on which authority will regulate this group.  I understand that this decision has been postponed pending the review of the HPCA Act.

12.  NURSING COUNCIL MEETING

In October I met with the Chair and CEO of the Nursing Council to discuss roles for nurses in perioperative care.  This was a very constructive meeting and I was able to clarify what the Nursing Council thoughts are on issues such as nursing scopes of practice and in particular the nurse practitioners’ scope, prescribing rights for nurses and roles of nurses in perioperative care.

13.  ANAESTHESIA BIBLIOGRAPHIES

The College Office in Wellington still has copies of the CD of Dr Basil Hutchinson’s “Bibliography of anaesthesia-related articles in the New Zealand Medical Journal, 1887-2000”.  This is available for $(NZ)13.50 + GST, or $(A)15.00.  Dr Hutchinson has now completed a “Bibliography of anaesthesia-related articles in the New Zealand Dental Journal, 1905-2006” and this is available on the NZ Dental Association website at:  www.nzda.org.nz/pub/resources/a_anaesthesiabiblio.htm

He has also compiled an Appendix which includes indexes of NZ authors of anaesthesia and related books, NZ anaesthetists with higher university degrees and obituaries of anaesthetists.  It is hoped that ANZCA will also publish the Dental Journal bibliography and the Appendix.  All three of these documents will be of interest to those delving into the history and development of anaesthesia and intensive care in New Zealand.

14.  OXFORD MINIATURE VAPORISER

Wanted - Oxford Miniature Vaporiser

Wayne Morris of Christchurch Hospital is planning to run a Remote Situations, Difficult Circumstances, Developing Country Anaesthesia course in Christchurch in early 2009.  The course will be similar to very successful courses run in the UK and Australia.

Part of the course involves hands-on teaching of drawover anaesthesia and he is currently looking for one or two Oxford Miniature Vaporiser (OMVs) and asks whether any department has an OMV.  He is also looking for a set of Oxford self-inflating bellows.
